Saw a guy at the Portland hammer-in using a 1/4 inch punch on a 3/4 inch hole. The steel just mushroomed.
People keep using punches that are way too small for the job, and it ruins the tool and the work. You need a punch that's at least half the diameter of the hole you're making, or you're just mashing metal. I learned this the hard way on a commission for twelve strap hinges. What's the biggest punch size mistake you've seen someone make?

How about the time I saw someone try to drift a hole with a center punch?

That's a classic move right there. Watched a guy use a screwdriver as a pry bar and snap the tip clean off. Some people just don't get how tools work.
