Rant: I just read the OSHA report on boiler accidents from last year and the number is way higher than I thought
I was looking up some code stuff online and stumbled on the full 2023 OSHA fatality and catastrophe report. It said there were 37 reported incidents directly involving boilers or pressure vessels. I always figured it was maybe a dozen. It really hit me that most of those were from things like improper lockout or missed cracks during an inspection. Has anyone else seen this report and know what the main causes were?
