After 11 months of fighting a shoulder injury from a warehouse job in Fresno, the claims adjuster finally said, "Your doctor used the wrong code on the RFA," and walked me through the fix in 10 minutes. Has anyone else had a denial come down to a simple paperwork mistake like that?
