Three weeks to get a simple prescription refill approved by my QME's office

I sent the paperwork for my lumbar MRI follow-up to the QME's office on the 3rd and they kept saying it was 'under review' until the 24th, and then they denied it because my attorney used the wrong form number, so now I have to restart the whole process, has anyone else had to fight this hard just to get imaging covered for their claim?

The QME's office lost my MRI referral twice last year for the same form issue.
