The double-wall pipe I swore by failed my draft test in Rutland

I've been using the same double-wall setup for 8 years, never had a complaint. Last month I did a job in Rutland and the draft was awful, barely pulling at all. Turns out the previous crew installed the inner liner upside down, which I didn't catch until I ran a smoke test. The whole thing looked fine from outside, but the baffle was blocking half the flue. I learned to check the directional arrows on every section before connecting, even if it's a brand I trust. Has anyone else found hidden install mistakes that only show up on a cold day?
