The 2 a.m. server rack fan failure that took down 12 clinic systems in Tulsa

I was on call last Thursday and the alarm went off at 2 a.m., found the cooling fan on the main storage array dead and the temp spiking to 118 degrees, I managed to swap it with a spare from the parts drawer before anything fried, but the whole time I was thinking, has anyone else had a fan die right after a firmware update or was mine just pure bad luck?
