Snapped a load line outside Tulsa last Tuesday, still replaying it in my head

I was setting steel beams on a school addition in Broken Arrow when the auxiliary hoist line let go at about 60 feet up, cable whipped like a fire hose and the beam swung maybe 15 degrees before I set it down hard. Nobody got hurt, but the tagline guy took a step back so fast he tripped over a rebar stake, and the foreman just stared at me without saying a word for a solid minute. I've checked the sheave and the drum brake twice since, but I still can't figure out if it was a bad spot on the rope or my boom angle, has anyone else had a line snap that never showed a warning on the daily inspection?
