My AI meeting notes kept inventing tasks, so I tried a different setup

I use an AI tool to transcribe and summarize my team's weekly calls for our project in Austin. Last month, it added a fake action item about updating a client deliverable that nobody ever discussed. It took me two days and a confused email from the client to figure out the AI pulled it from an unrelated old thread. I switched to feeding it only the raw transcript and turned off the auto-summarize feature. Now I review the raw text first, then ask for a task list with timestamps. Has anyone else caught their AI tool adding stuff that never happened on a call?

Tech is great until it starts lying, that's the whole problem.

Stumbled onto this thread late but had to jump in @amy154 because I dealt with this exact thing last month. My phone kept auto-correcting my texts to say stuff I never typed, like changing "got it" to "good it" and once it turned "dinner" into "diners" which made my wife think I was going out with coworkers instead of coming home. What finally worked for me was turning off the predictive text and the smart reply feature altogether, and I also stopped using the voice-to-text thing after it wrote "I'll be there at 8" as "I'll be there at ate". It's annoying because the convenience is nice, but I'd rather type everything out slowly than have the tech put words in my mouth. It still messes up sometimes but at least now I know when it's lying to me.
