My AI note taker kept inventing meetings I never had

Last week I used an AI transcription app for a client call in Tampa and it added a whole follow up meeting that never happened. It even put fake action items in my calendar, I only caught it because my coworker asked about a deadline I knew nothing about. I spent two hours untangling the mess and checking every note against the original recording. The tool is supposed to save time but it made me double check everything anyway. Has anyone else had AI summaries fabricate details like this? I'm thinking about going back to typing notes manually.
